account debit card. Your employer offers the mbi
Flex Convenience® card as an easy
way to access the funds in one or more of the following
benefit programs: • Flexible Spending Accounts
• Dependent Care Accounts
• Health Reimbursement Arrangement Accounts Your mbi Flex Convenience® card will
work anywhere MasterCard® or VISA® is accepted
for your eligible plan expenses. No more double out of
pocket expenses, filling out claim forms, or waiting for
reimbursement checks! No PIN or access code to remember! Using your mbi Flex Convenience®
card is as easy as 1-2-3: 1. Sign the back of your card
before using.
2. Present your card to purchase qualified goods and
services (as defined by your plan administrator).
3. Save all receipts. If requested by your plan administrator, you are obligated to submit your card transaction receipts and/or any other related claim information and documentation as deemed necessary to substantiate expense eligibility.
It is important to note that virtually every
employer applies certain rules and restrictions to the
use of FSA debit cards, particularly with regard to larger
reimbursement transactions. Please consult with your plan
administrator to see which rules and restrictions apply
to your mbi Flex Convenience® card.
